Residing in the Chattanooga area gives you a unique advantage, because you're able to walk-in to our office for services.
Internet Customers-
How do I schedule my blood work?
Simple.
1). Call Lab Plus @ 423-643-0700, or email us at CSAMedical@yahoo.com
2). Provide us with your zip code, so we can find the nearest patient service center to you.
3). Once appointment is scheduled we provide the necessary order, and you provide the specimen. You must have a lab order form before the drawing is done.
4). A national laboratory will complete the test and send the results to Lab Plus. Blood tests are analyzed by board certified MD's.
5). Lab Plus physicians will review your test results.
6). Results 24-48 hours in most cases.
The healthcare consumer will choose the return of results by:
Fax
Pickup
U.S. Mail
Confidential Secure E-mail.
If you want a copy sent to your healthcare provider, you must sign (HIPAA) form and fax to 423-643-0701
(HIPAA form must be placed on line to be printed in English or Spanish)
